Credit Memo Authorization (CMA) online form
If you would like to request a product return or a exchange, you are required to have a Credit Memo Authorization (CMA). Please complete and submit the online form below and include the following information in the "Details" field.
- Original invoice# of the item(s) to be returned or exchanged
- The item number
- The item description
- The item quantity
- Reason for return or exchange
- State the condition of the product. NEW - undamaged and in original packaging; OPENED - product like new but not in original packaging; DAMAGED - product damaged or missing parts.
Once approved, the GTI office will email you an approved CMA via that you or your staff can use to return product for store credit. Once returned product has been received by GTI staff along with the approved CMA, your credit will normally be processed within two business days.
